Please note that a physician’s signature is not required on the Health Form. However, you must fill out the first page and sign it. The requirements from the NYSDOH are a parent’s signature and the immunization records. A photocopy of the immunization record is acceptable and may be attached to this form or scanned and sent to this email address.

The DOH may be auditing these records, so please return your form no later than the Friday before your child's session begins. If you plan to bring health forms with you on your child's first day of camp, please let me know in advance.
