Camper Health Form
Camper Health Form
A physician’s signature is not required on the Health Form. You must fill out the first page and sign it. The requirements from the New York State Department of Health are a parent’s signature and the immunization records. A photocopy of the immunization record is acceptable and may be attached to this form.
The Department of Health may be auditing these records, so please return your form no later than the Friday before your child's session begins.
